Mental Health Outreach Mobile Clinic / Emergency Homelessness & Shelter Support Services
CLCS runs a specialised outreach mobile nurse-led depot clinic administration services.
The clinic is an enhanced primary care in mental health, with a focus on relapse prevention, rehabilitation and quality of life.
The clinic supports with long acting injectable (LAIs) administration-which has the ability to slowly release desired amount stretched over an extended period of time.
We welcome referrals from In-patient and community public mental health services, private psychiatrist services, GPs, and NGOs.


Primary target population who may benefit from our outreach nurse-led depot clinic are clients:

With failed GP discharge

Staying in step up/down accommodations

Staying at 24/7 supported accommodations

Requiring home visits

Requiring 2-hours post monitoring

Requiring After hours depot support

Not keen on GP discharge

Not keen on continuing depot with public MHS

With no case management needs except depot
-
IPU discharge, on depot with no case manager allocation needs

Interstate visitors requiring depot support whilst visiting
